First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. A preoperative consultation and physical examination is mandatory for patients prior to surgery. In most cases, the surgery will be scheduled for another day at your convenience. Occasionally, surgery can be performed the same day as the consultation, especially in an emergency situation.
After Dr. Owsley or Dr. Brooksbank sees you and determines your treatment plan, our administrative staff will discuss the estimated costs of your surgery. They will check with your insurance company and give you an estimate of what portion of your surgery will be covered by your insurance company.In many cases, a pre-authorization of the proposed procedure can be obtained prior to the surgery.
You will be asked to complete a patient registration form on your first visit which includes a medical history and list of medications. If you would like, you may go to the “Patient Registration” section of this website and print the registration forms. You can then complete them at your convenience and bring them to the office when you come for your consultation.
Please assist us by providing the following information at the time of your consultation:
— Your patient registration form if you have completed it before coming for your initial visit
— Your surgical referral slip and any applicable x-rays
— A list of your current medications
— Any medical or dentalID cards – this will allow us to help you process any claims
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
Please inform the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
